- Copy of applicant's citizenship passport and residence permit copy 2 attached
- 4x5 Size (Background) white photo
- Service charge 1,125 SR
Ethiopian by Parent (Yellow Card)
- Original (copy) of Ethiopian passport of mother/father
- Birth Card attested (translated in English by Minister of Foreign Affairs)
- Original and 2 copies of applicant's valid passport
- If you are under 18 years of age, the fee is SR 375
- For lost adult's Ethiopian Origen ID 1688 SR
- Correction of identity card for adults of Ethiopian Origin ID /yellow card/ Correction 1313 SR
- For children's Ethiopian origin ID data correction , Complete proof of correction and 563 SR
- Lost children's Ethiopian origin ID 938 SR
